Summary of critical evidence: Portal and splenic vein thrombosis are common complications of AP, which may lead to regional portal hypertension [80]. In a single-center retrospective study including 825 patients with moderately severe and severe AP, of whom 541 received warfarin or low-molecular-weight heparin (LMWH) during hospitalization, early prophylactic anticoagulation was not significantly associated with the occurrence of PPH [47]. Therefore, current evidence does not support routine anticoagulation to prevent PPH in AP. Another retrospective study reported that early systemic anticoagulation significantly reduced the incidence of splanchnic vein thrombosis in patients with acute necrotizing pancreatitis, and was associated with lower rates of organ failure and in-hospital mortality without increased risk of bleeding complications [81]. Thus, anticoagulation may be considered to prevent splanchnic thrombosis in acute necrotizing pancreatitis, but its benefits on the prophylaxis of PPH remain unclear
